When referring to pharmaceuticals, we have used generic names whenever possible. Proprietary (product) names have been used when the length or complexity of the generic name would be unwieldy or potentially confusing.

Sl (International System) unit values are shown on occasion in brackets and italics following the listing of conventional unit values.

Throughout this book we have used the female gender for nurses, the male gender for doctors, and the male gender for patients of both sexes, unless specific examples require otherwise. This avoids the cumbersome use of both genders whenever nurses, doctors or patients are mentioned. We have chosen this usage because, in the United States at present, the majority of nurses are female, and the majority of doctors are male.
